Before the visit
- Send 1 week and 24-hour reminders with a note to fill out the Pre-Visit survey before the visit. REDCap will automatically send these to their email
- Review the Pre-Visit survey to understand the student’s asthma.
- If they did not complete these forms on their own, you will have to help them complete them during the visit
At the visit, discuss:

Exercising with asthma
Medication review
Inhaler technique review
Wrap-Up
Summarize Visit
Let them know they may be contacted to complete a feedback survey or interview.
Ask the family what type of gift card they would like to receive as a token of appreciation and enter it into your visit log.
After the visit
Log info into REDCap
End of school year summary to HCP
