Review health information submitted by families
Identify children who indicated they have asthma or use inhaler but did not complete forms
Review Asthma Intake Forms for completeness
Review Colorado Asthma Care Plan and Medication Order for Schools and Childcare Settings Plan for completeness

Children meeting any of the following criteria are considered to have at-risk asthma.
☐ 2 or more urgent care/ER visits for asthma in the past 12 months
☐ Hospitalization for asthma in the past 12 months
☐ 2 or more courses of prednisone or systemic corticosteroids for asthma in the past 12 months
☐ More than 5 missed school days due to asthma in the previous school year
☐ Uses rescue / quick relief medication more than 2 days per week
☐ Has daytime asthma symptoms more than 2 days per week
☐ Awakes at night with asthma more than 2 times per month
☐ Physical activities are often or always limited
