Gather Asthma Care Plans and Medication
Meet student needs quickly by having medications and care plan at school.

Help kids focus on schoolwork, not asthma!
Obtain an asthma care plan for each student
- Ask parents to fill out this form and return it to school: Colorado Asthma Care Plan and Medication Order for School and Child Care Settings.
- Form in more languages
- Review completed CO Asthma Care Plan
Work with caregivers and health care teams to get care plans from providers
Prioritize:
- Students with features of high- risk asthma based on the Asthma Intake Form
- Student who regularly present to the office with breathing problems.
Ensure each student has access to needed asthma medications
- Ensure a reliever inhaler is available at school.
Alert families and providers on asthma control, based on what they see
