This visit with the family will take around an hour and half to complete.

Before the Visit: Remind and Review

Send a reminder (email, text, or voicemail) that navigators and family members have agreed to meet at a specified time and place.

Review AIF to identify eligibility criteria that will be confirmed during the visit.

At the Visit:

Important Note For the WAITLIST GROUP:

• Do not complete: SDOH Conversation, asthma education, or a student visit

Wrap-Up

Summarize visit and thank them for their time

Confirm with the family that they will get an electronic gift card in their email and report the visit complete in the Visit Log.

The study team will send them a post visit survey which will inform them that they are waitlisted and a navigator will reach out at the end of the school year to continue with the program.
