In the Torreys group, you will be working closely with Regional Health Connectors (RHC) from Trailhead to link families with resources.
This collaboration is meant to create a relationship between the community and you as a navigator which might help you better assist families.
RHC and Asthma Navigator Collaboration Plan
- Connect annually in June/July for warm handoffs to at least 6 community resources
- RHCs will join a few of AN meetings per year
- RHCs have helped identify top resources in the Resource Guide
- Create a plan for the 6 warm hand offs (shared phone call or in person meeting with organization preferred)
- More introductions (if needed) and get to know each other
Discuss resource list and orient ANs to resources in the community
IMPORTANT SDOH Contact Guardrails:
- Discussion of RHCs (including warm handoffs and anything else) will stay within Torreys
- Share RHC questions in the warmup (and/or debrief)
- Use the SDOH Contacts Form in REDCap
REDCap
